HACC Program for younger people (HACCPYP)

HACCPYP provides services for people with disabilities and their carers. Younger people are defined as those aged under 65 and aged under 50 for Aboriginal people.

HACCPYP provides basic support and maintenance services to help people who need assistance to remain living at home as independently as possible by providing a range of services to support them in their own home.

How can I be referred?

You may refer yourself for HACCPYP by calling Council.  Referrals may also be received from doctors, hospitals, family and friends (with your consent).

Once the referral has been received we will contact you to see if you are eligible and arrange a time to visit you to discuss your needs.
