| What service does Council provide? Loddon Shire Council provides Home and Community Care (HACC), a program that supports older people to remain in their own home for as long as they are able and wish to do so.
HACC provides subsidised home-based services, such as: * Food Services: including delivered meals, a meal in a community facility, or help with shopping * Home Care: such as cleaning, cooking, washing and ironing * Personal Care: help with bathing and dressing * In Home respite: providing a break for carers * Short term support: to assist people following discharge from hospital * Home maintenance : outdoor maintenance such as lawn mowing, cleaning house guttering or minor modifications, such as: replacing tap washers, replacing batteries in smoke detectors and installing ramps and rails * Planned Activity Groups: Social groups that include meals, activities and outings. Limited transport is available to group, social activities and medical appointments. | |
| Who is eligible? People living at home who: * Have a disability * Are older and are assessed as needing support * Are carers of those people.
What is an assessment? A staff member from Council's Aged and Disability team will meet with you in your home to determine if you are eligible for the service, your level of need, priority for service and individual cost.
What is the cost? Charges range from minimal to full cost, according to your ability to pay. No person will be denied a service because thay are unable to pay.
Who can refer? * Self * Doctor * Family, friend or neighbour * Other health and community services * Aged Care Assessment Service (ACAS)
Is my personal information kept confidential? All details provided at assessment and the details of your service provision are kept strictly confidential.
What are my rights when using the service? The Loddon Shire Council HACC service acknowledges the client's rights and responsibilities. The right to: * Dignity and privacy * Access to service on the basis of need * Be well informed about support options and cost * The right to make your own decisions * The right to complain and have the complaint dealt with quickly and fairly.
The responsibility: * To participate in decision-making * For the outcomes of your decision * To speak up if you are not happy with the service provided * To afford respect and dignity to staff and other clients.
What about more complex health needs? People with high levels of care needs may still wish to remain at home and receive co-ordinated care. Community Aged Care Packages or the Linkages Program may be needed. For further information contact Bendigo Health on 5454 7588 or 5454 7589.
What if I am a Veteran? The HACC program assists assessed veterans and war widows/widowers who have health care needs and require support to remain living in their own home.
What if language is a difficulty? Contact the Translation and Interpreter Service (TIS) on 131 450 and ask them to call the senior provider or Seniors Information Victoria.
